Where can you obtain immunizations?



Contact your medical provider.  This is a great opportunity to schedule a well child check if your child needs one.


If you are unable to schedule with your medical provider, immunizations are also available at:



Aviva Health (Previously known as Umpqua Community Health Center)

  • Aviva Health – Roseburg – 150 NE Kenneth Ford Drive (next to Costco Gas)


Monday through Friday from 8:30 am to 11:30 am and from 1 to 4 pm

  • Aviva Health – Sutherlin – 123 Ponderosa Drive (next to Apple Peddler)


Monday through Thursday from 1 to 4 pm

  • Aviva Health – Drain* – 316 West A Avenue 

Friday from 8:00 am to 11:30 am and from 1 to 4:30 pm

*(For Drain call the Sutherlin office phone number to confirm availability)

  • Aviva Health – Myrtle Creek – 790 S. Main Street


Monday, Tuesday, Thursday, Friday from 1 to 4 pm



Dunes Family Health Care, Reedsport

Located at 620 Ranch Road

Monday through Friday from 8 am to 12 pm and 1 pm to 5:30 pm

Saturday 9:00 AM to 12:00 PM

Pharmacies can immunize children 7 years and older without a prescription, or younger with a prescription.  Parents should contact their neighborhood pharmacy for availability of vaccines and details.

Important: Please note that children on UHA/DCIPA or DMAP cannot be vaccinated at a pharmacy. They must get their immunizations at a clinic.


No one will be turned away for their inability to pay at Aviva Health or Dunes Family Health Care.
